OVERVIEW
The Flexible Benefits Program lets you decide if you
want accidental death and dismemberment (AD&D)
insurance.
You may choose coverage equal to:
- one times your pay
- two times your pay
- three times your pay
- four times your pay
- five times your pay
- six times your pay
- seven times your pay
The coverage maximum is $500,000.
In general for the AD&D insurance to be paid to you or your
beneficiary, your death or injury must be the result
of a covered accident. In case of permanent and total
disability, you are eligible for AD&D benefits
if your injury prevents you from working at any job
for which you are qualified by education, training,
or experience.
ENROLLING FOR COVERAGE
Your cost depends on how much AD&D insurance you
choose. As with life insurance, your coverage will
be based on your Benefit Salary as of October 1, 2007
or your date of hire, whichever is later. This amount
is rounded to the next higher $1,000, after you multiply your
pay by your election. If you are age
75 or older, the value of your coverage is reduced.
The beneficiaries you name for life insurance are also your
beneficiaries for AD&D insurance. If you are not enrolled in life
insurance, be sure that you complete the Flexible Benefits Beneficiary
Election Form to name your Beneficiaries. You can change your beneficiaries
anytime by completing a new form. Be sure to return the completed form
to your personnel office.
